ABSTRACT

A patient, diagnosed with ulcerative colitis and rheumatoid arthritis and treated with mesalazine and hydroxychloroquine, presented with symptoms of catarrh similar to those of COVID-19. The pharmacotherapeutic monitoring service (PMS) provided to the patient allowed us to associate these symptoms with a side effect of the drug taken by the patient, i.e., blood dyscrasias. The intervention, carried out to inform the doctor while the patient was symptomatic, allowed the HCP to modify the treatment by reducing the doses of the drugs used until the condition was under control.

ABSTRACT

The objective of the research was determining how affective pedagogy stimulates meaningful learning in students of the Faculty of Administrative and Economic Sciences, Católica University of Trujillo Benedict XVI (UCT-FCAE) in health emergency times. With a non-experimental quantitative approach, descriptive causal correlational transactional and explanatory scope. The sample was 57 students from the 2020-I semester of the UCT-FCAE. As an instrument, a questionnaire for Affective Pedagogy and another for Significant Learning were used, both were validated through Aiken’s V and Confirmatory Factor Analysis, with reliability through Cronbach’s alpha. It was found that affective pedagogy, love for others, love for the world and knowledge, significantly favor significant learning in (r2 = 20.5%;r2 = 57.9%;r2 = 35.2%;with p <0.01). Meanwhile, self-love favors it in a non-significant way (r2 = 4.8%;with p <0.05). © 2021, Associacao Iberica de Sistemas e Tecnologias de Informacao. All rights reserved.

ABSTRACT

Mirizzi syndrome (MS) is a rare complication of gallstone disease. Surgical management may be complex, depending on the type of Mirizzi, and may range from simple cholecystectomy to a hepaticojejunostomy. A 39-year-old female diagnosed previously to have gallstones was admitted in the Philippine General Hospital (PGH), a tertiary COVID 19 referral center presenting with obstructive jaundice, right upper quadrant pain and fever. Hepatobiliary ultrasonography demonstrated a markedly distended gallbladder with a stone at the Hartmann's pouch. A diagnosis of Mirizzi syndrome with cholangitis was made. The patient was managed with broad spectrum antibiotics and scheduled for surgery. No further imaging or endoscopy was requested due to the limitations brought about by the pandemic. Upon a negative COVID 19 polymerase chain reaction (COVID 19 PCR) test result she underwent surgery, subtotal cholecystectomy, intraoperative cholangiography, and choledochoplasty. No perioperative complications were noted. This patient patient care while limiting risks from COVID 19 both to the patient and the health care team. This case demonstrates the need to address issues brought about by the pandemic resulting in limitations in resources, added risk of morbidity to patients, and increased level of technical difficulty particularly in biliary surgery.
